Home Health Aide (HHA) / Personal Care Aide (PCA)

Platinum Home Health Care is seeking compassionate, reliable, and tech-friendly caregivers to provide high-quality support to clients throughout New York. As part of our team, you’ll help clients remain safe, independent, and comfortable in their homes.
Duties & Responsibilities
Personal Care & Daily Support
- Assist clients with personal care tasks such as bathing, dressing, grooming, toileting, and hygiene, as permitted by your certification (HHA or PCA).
- Support clients with mobility, walking, positioning, and safe transfers (including use of mobility equipment if trained).
- Provide companionship, emotional support, and client engagement.
Home Management & Routine Tasks
- Assist with light housekeeping, laundry, meal preparation, and general household tasks that help maintain a safe environment.
- Support with shopping, errands, and meal planning as outlined in the Care Plan.
Health Observation & Reporting
- Observe and promptly report changes in client condition, appetite, mobility, behavior, mood, or skin integrity to your supervisor or nurse.
- Follow the Care Plan exactly as assigned by the nurse and agency.
- Maintain infection-control practices and universal precautions.
Technology, EVV & Documentation
Because NYS Medicaid requires Electronic Visit Verification (EVV):
- Use the HHAeXchange app (or approved EVV system) to clock in/out and complete visit tasks.
- Review schedules, case updates, and communication through your mobile device.
- Communicate availability, case acceptance, and changes through app, phone, or agency portal.
- Complete digital documentation and required forms accurately and on time.
- Complete online in-services and compliance modules as assigned.
Training is provided.
Professionalism
- Arrive on time and follow all agency attendance and conduct standards.
- Maintain respectful communication with clients, families, and agency staff.
- Protect client privacy and follow HIPAA guidelines.
Qualifications Required
- Valid New York State HHA or PCA certification (DOH-approved).
- Must be 18 years or older.
- Ability to read, write, and perform basic math for documentation and daily tasks.
- Comfort using a smartphone and mobile apps (EVV, schedules, communication).
- Ability to follow instructions, Care Plans, and agency policies.
- Ability to safely perform physical duties, including lifting, bending, standing, and transferring clients.
- Professional, dependable, and compassionate attitude toward client care.
Preferred (Not required)
- Experience supporting clients with dementia, post-surgical needs, or chronic conditions.
- Bilingual
- Strong communication and comfort using texting or portals for updates.
